Refer a patient or student for ADHD testing
Comprehensive two-session ADHD evaluations for children ages 6 and up, adolescents, and adults, in Grand Rapids and Byron Center, Michigan, and virtually statewide. Typical time from referral to first appointment is two to four weeks. Reports are returned to the referring provider with patient consent, usually within two weeks of the second session.
What the evaluation includes
One-hour structured clinical interview covering developmental, medical, academic, occupational, and psychiatric history. Approximately two hours of standardized testing including the Test of Variables of Attention (TOVA) and age-appropriate validated rating scales with collateral informants (parent, teacher, partner). Differential diagnosis for anxiety, depression, trauma, sleep disorders, and specific learning disorders. A written report with DSM-5-TR diagnosis and presentation type, instruments and scores, functional impact, and specific recommendations. A one-hour feedback session with the patient or family.
What the report supports
Medication decisions by the prescribing physician or psychiatrist. Section 504 plans and IEP eligibility meetings. College and graduate school disability accommodations, including documentation standards for the LSAT, MCAT, GRE, and licensing boards. Workplace accommodation requests under the ADA. FMLA and disability paperwork where relevant.
Who is a good fit
Patients with suspected ADHD who need a diagnosis that will hold up with schools, universities, and insurers. Patients with a prior brief diagnosis who need a fuller workup before or after a medication trial. Adults with unclear histories where anxiety, depression, or trauma may be driving attention complaints. Children and teens whose school performance does not match their ability.
Not a fit: patients under age 6, acute psychiatric crisis, or neuropsychological evaluation for dementia, TBI, or seizure disorders; those are referred on.
How to refer
No referral form is required. Have the patient or parent call the office nearest them and mention your name; a signed release will be obtained so the report can be sent to you. If you prefer to send a referral directly, fax or email a brief note with the patient’s contact information, reason for referral, and any prior testing to aw@drandrewwichterman.com. Please do not include protected health information in unencrypted email beyond name and contact details.

Grand Rapids (Centennial Park Counseling): Aetna, Blue Cross Blue Shield, Blue Care Network, ASR Physicians. Self-pay with superbill is available at both. Some plans require prior authorization for psychological testing; the office staff will confirm before scheduling.
