Adult ADHD: How It Works and What Helps

Adult ADHD rarely looks like the cartoon. It looks like a competent person who is exhausted by things other people find easy: starting, finishing, remembering, arriving on time, keeping a room clear, keeping a bank account balanced. This collection explains the mechanisms behind those experiences and what to do about each one.

How adult ADHD is different from the childhood picture

Hyperactivity fades or turns inward with age; what remains is the executive function deficit. Adults with ADHD describe restlessness rather than running, and their impairment shows up in the tasks adulthood piles on: bills, appointments, email, laundry, long projects with no one checking. The diagnosis requires that symptoms started before age 12, which is why an evaluation spends so much time on childhood history even for a 45-year-old.

Why it gets missed for decades

Intelligence, a structured household, a supportive partner, or a career that rewards hyperfocus can mask ADHD for years. It surfaces when the scaffolding falls away: a promotion into management, a first child, a divorce, remote work. Women are missed more often because the inattentive presentation is quieter and because anxiety and depression, both common companions, get diagnosed first.

What a diagnosis changes

Access to medication, which has the largest effect size of any psychiatric treatment. Workplace accommodations under the ADA. An accurate explanation for a lifetime of "not living up to potential," which for many adults is the first relief from shame. And a framework for choosing strategies that fit the mechanism rather than trying harder at strategies built for a different brain.
