Managing ADHD Without Medication

Medication is the most studied ADHD treatment and, for many people, the most effective. It is also not the whole picture, and it is not available or acceptable to everyone. These posts sort the non-medication options by evidence rather than by enthusiasm.

Some of what is here is well supported: aerobic exercise, sleep treatment, externalized structure, and behavioral parent training for children. Some of it is honest about weak evidence, including most supplements. All of it is written to be used, with specific strategies rather than encouragement.

What the evidence actually supports

Aerobic exercise has consistent, moderate effects on attention and mood. Treating sleep disorders often improves attention more than any supplement. Behavioral parent training is the best-supported non-medication treatment for children. External structure, meaning alarms, lists, body doubling, and environment design, works because it supplies what internal regulation does not. Cognitive behavioral therapy adapted for ADHD helps adults, especially with the shame and avoidance that build up over years.

What the evidence does not support

Most supplements, with omega-3s showing a small effect and everything else showing less. Elimination diets for the general ADHD population. Brain-training apps as a standalone treatment. Willpower, effort, and "just focus," which are the interventions most people have already tried for decades before they arrive in my office.

How to think about medication in this picture

Stimulant medication has the largest effect size of any treatment in psychiatry. That is a fact, not a recommendation. Many people cannot take it, do not want it, cannot get it during shortages, or want to try structure first. Every strategy on this page works alongside medication and works without it, which is why it is worth learning regardless of what you decide.
